Who qualifies

You must be an active duty service member, and one of these has to be true.

  • Orders that may come with hazardous duty pay

You have orders, or expect to get them within the next 120 days, for duty that may come with hazardous duty pay.

  • Orders that separate you for 180 days or more

Your orders will separate you from your spouse, domestic partner or dating partner for at least 180 days. Sea duty counts.

Timing matters. If your orders may come with hazardous duty pay, your egg freezing has to be finished before your orders begin, so start with consultations and testing as early as you can. If you are facing a separation of more than 180 days, you can complete it before you leave or during the separation, if the care is available where you are assigned.

Spouses, retirees and veterans are not eligible. Your commander confirms your eligibility by signing DHA Form 459.

  • Freezing your eggs

Our military package

For active duty service members who have been approved for reimbursement, we offer a military package on the egg freezing cycle and the freezing fee, with one year of storage included. It is built to keep the cycle, freezing and typical medication costs inside the program’s $10,000 yearly limit.*

What the package includes

  • Your egg freezing cycle
  • Freezing your eggs
  • One year of storage
  • Paid separately: medications, bought directly from the pharmacy

*Package pricing is available only to active duty service members who have been approved for reimbursement through the DoD Cryopreservation Demonstration Program. Medications are not part of the package. They are paid directly to the pharmacy and the amount varies from patient to patient, so your total cost can come in above the $10,000 limit. Ask us for a cost estimate before you start.

How it works

  1. Get in touch

Send an appointment request or call the office. Tell us when your orders begin. If they may come with hazardous duty pay, your egg freezing has to be finished before they start, so earlier is better.

  2. Confirm you qualify

Download DHA Form 459 and have your commander sign it. The signature confirms you were eligible when you received your care.

  3. Consultation and testing

You meet with Dr. Jacob. Before the retrieval we complete a checklist: infectious disease testing, hormone monitoring, follicle tracking, consents and injection teaching.

  4. Egg retrieval and freezing

Eggs are retrieved by ultrasound guided follicle aspiration while you are under light sedation, then frozen in our laboratory.

  5. Pay, and get your paperwork

You pay upfront. We give you an itemized invoice with your dates of care, medical codes and costs, and a statement showing a $0.00 balance, which is what the program asks for.

  6. File within 120 days

Email DHA Form 459, Standard Form 3881, your invoice and your proof of payment to the program within 120 days of your egg retrieval. Late requests are not reimbursed.

A military hospital, or a civilian clinic

The program lets you use a participating military hospital or a civilian fertility clinic, and a civilian clinic does not have to be TRICARE-authorized. The military hospitals that offer this care are in Maryland, Texas, Hawaii, California, North Carolina, Virginia, Washington and Ohio. Care there is offered as space allows, and they may not be able to schedule you around your orders.

At GoFertility you will see Dr. Saji Jacob at every key appointment. He is a board-certified OB/GYN, reproductive endocrinologist and infertility specialist, and a trained embryologist. Egg freezing happens in our own CAP-certified laboratory, and Newsweek has ranked us a top-50 IVF clinic in the country since 2023, one of just two Missouri clinics to receive that designation.

Questions we are asked often

Is this a TRICARE benefit?
No. The Defense Health Agency says the Cryopreservation Demonstration Program is not a TRICARE program, and TRICARE itself does not cover cryopreservation. It is a separate Department of Defense reimbursement program.
Does my egg freezing have to be done before I deploy?
If your orders may come with hazardous duty pay, yes. The Defense Health Agency says your procedures must be finished before your orders begin. If you are facing a separation of more than 180 days, you can complete them before you leave or during the separation, if the care is available where you are assigned. Storage and later shipping costs are not held to that cutoff.
Can my spouse or partner use the program?
No. Only the active duty service member can be reimbursed. Spouses, retirees and veterans are not eligible.
Do I have to use a military hospital?
No. You can choose a participating military hospital or a civilian fertility clinic, and a civilian clinic does not need to be TRICARE-authorized.
What if my costs come to more than $10,000?
The program reimburses up to $10,000 a year for egg freezing, and anything above that is your cost. Medication is the part that varies most from patient to patient, so ask us for a cost estimate before you start.
Does the program cover IVF or embryos?
No. Embryo creation, in vitro fertilization and other elective procedures are not reimbursed. The program covers freezing your eggs, not using them.
Where are my eggs stored, and for how long?
The military package includes one year of storage. Our cryo storage bank is meant for shorter term storage. If you want long term storage, there are several storage banks that may be more cost-effective, and we can talk through the options. The program reimburses shipping and storage, but not new storage or shipping costs after Sept. 14, 2029.
When do I have to file for reimbursement?
Within 120 days of completing your egg retrieval. Requests that arrive late are not reimbursed.
I froze my eggs before Sept. 14, 2026. Is that covered?
No. Care before Sept. 14, 2026 or after Sept. 14, 2029 is not reimbursable.
Will using the program affect my military benefits?
No. The Defense Health Agency says taking part will not affect your eligibility for TRICARE or other military benefits.
What about freezing sperm?
The program reimburses sperm freezing up to $500 a year, and our cryo bank offers sperm banking. Call the office to talk it through.
